The Haverhill Selectboard Office requests your assistance in updating our records as well as confirming your eligibility for the religious, educational, charitable tax exemption; which, per State Law (RSA 72:23-c & 72:23, VI), is to be confirmed for qualification every year. Due to the recertification process, mandated by the State Division of Revenue Administration, towns must now require their non-profit, charitable organizations to submit this paperwork.

Please find enclosed the two part BTLA Form A-9 as well as the BTLA Form A-12. Both need to be filled out and the originals returned to our office no later than April 15, 2006.

The Haverhill Selectboard Office requests your assistance in updating our records confirming your eligibility for the veterans’ tax exemption; this is a requirement of the certification process mandated by the State of NH.

Enclosed you will find PA 29 (green) form. This form needs to be filled out and returned to our office no later than April 15, 2006. We have highlighted the fields on the form which need your attention and please do not forget to sign.

We also need to verify your honorable discharge and service dates via your certificate of release or discharge from active duty (Form DD214). If you do not have a copy, we have contacted the NH VA in Concord; Jackie Page (225-1202) or Pete Fortier (225-1380) will be able to assist you in obtaining it.
